The fifth principle is where Loguaron stops collecting and starts interpreting, carefully. The danger here is overreacting to a single data point, or dismissing a real trend as noise. Analysis means asking whether what you are seeing is ordinary variation or a genuine signal, and judging each clinic against its base rate rather than your most vivid memory of it.
What the science says
Two bodies of evidence anchor this principle. The first is the statistics of variation. In Statistical process control as a tool for research and healthcare improvement (Quality & Safety in Health Care, 2003), Benneyan, Lloyd, and Plsek explain that every repeated measurement naturally differs, and that without a method to separate normal “common-cause” variation from a true “special-cause” signal, improvement efforts chase noise. A single bad month may mean nothing; a sustained shift across many points means something.
The second is the psychology of misjudged frequency. The availability heuristic identified by Tversky and Kahneman (1973), and the neglect of base rates synthesised in Kahneman’s Thinking, Fast and Slow (2011), explain why unaided judgment is so unreliable: we over-weight the dramatic clinic we remember and under-weight the steady one we don’t. Structured analysis corrects exactly these distortions.

This principle lives in the Monthly Review, where clinics are compared on workload, activity, procedures, administrative burden, payment reliability, and friction. The built-in ladder mirrors the statistics: if a problem appears once, observe; if it appears repeatedly, investigate; if it becomes a pattern, act. That escalation is a manual control chart. The question it answers: Which clinic creates value, and which creates friction?
