Observe the System

The second principle asks you to lift your eyes from the incident to the system that produced it. A hard week is an event. A clinic that produces hard weeks is a structure. Most clinicians spend their attention on events (the difficult case, the staffing gap, the late lab) and almost none on the structure quietly generating them. What the science says

Systems thinking provides the language. In Thinking in Systems: A Primer (2008), Donella Meadows draws the central distinction: beneath the visible stream of events lie patterns of behaviour over time, and beneath those lies the system structure that drives them. Reacting to events, she argues, is the lowest-leverage place to stand, because the same structure keeps generating the same events no matter how skilfully you handle each one.

Peter Senge made the same case for organisations in The Fifth Discipline (1990), where systems thinking is the discipline that integrates all the others. His core warning is that people trained to look for proximate causes (a specific person, a specific day) routinely miss the systemic structures that make those events almost inevitable. “Structure influences behaviour”: put different people into the same system and they tend to produce similar results.
